I'm sorry if this is an obvious question, but do I need to fill out i-864 from or the I-134 form for affidavit of support?

I-864.

There are various types depending on your condition. I am filling out a I-864EZ as an example.

May I suggest further readings of the guides?

The i-864 form says in the top expires: 9/2012. I can still fill that out and send it?

They accept expired forms yes.

You can read Here: http://travel.state.gov/visa/immigrants/info/info_3197.html and find out which you could/should be using. My husband filled out an I-864EZ because he met the 3 conditions for the form.

Your question indicates you have not yet carefully studied the CR1 visa guide OR the I-864 instructions. Doing so, will be critical to your ultimate success.

There are various types depending on your condition. I am filling out a I-864EZ as an example.

May I suggest further readings of the guides?

The OP indicated they needed a cosponsor (really a "joint sponsor") so clearly, the I-864EZ does not apply.

Sorry guys, I had already read the above mentioned guides. I just got side tracked, because I went to print out the I-864 form, then suddenly I noticed the expiration date.

So then I did a little research and ended up on the AOS I-134 form page, then I got confused again because I have never seen this form's name. Which is why I decided to come here.

Sorry, I just want to go to the NVC process as fast as I can, which means making no mistakes!

if you have a joint sponsor, yes you have to file the I-864. Do not worry about the expiration date.

The expiration date on the form is the expiration of the comment period to the Office of Management and Budget. The form itself does not expire; therefore, if that is the only form available, it can be used.

I read what you wrote. This part was irrelevant, "There are various types depending on your condition. I am filling out a I-864EZ as an example.", BECAUSE the OP had already described enough of their "condition".
