I'm finally filling out my last form for packet 3 to be sent back.

I have a tricky question. On the DS-157 form,when i have to put down supervisor contacts for my last 2 employers, i'm a bit unsure. From 2008 until the beginning of 2011 I have had different job roles through a recruitment agency, they have all been with the same employer. My question is, i have a contact number of my last supervisor for my last job role but what would be the contact before that? Would it be the job role i had before that which was the same employer, or i get or would i have to get a supervisor contact from the employer i was with before i started the agency work? Do i need to put the agency contact number as well?

I do need help with this, thanks
