K-2 Question

Okay this is a pretty simple one. But I want to make sure I have everything in order for when I get to the interview stage.

My 13 yo daughter is coming with me to the US to live, her father is fine with this and is also fine with signing a consent letter.

My question is, is there any special layout that has to be done with this letter/affidavit or wording?

This is what I have done, I intend to attach a copy of his drivers license with current address:

AFFIDAVIT

I, XXXXXXXXXX of XXXXXXX, XXXXXX in the State of XXXXXXX, Australia, make oath and say as follows:

1 I am the father of XXXXXXXXXX born XXXXXX, 1992.

2 I know that my former wife, XXXXXXXXXX, intends to take my daughter XXXXXX with her to live in the United States of America.

3 I hereby give consent to this occurring.

Any suggestions from anyone that has done this with Australia would be greatly appreciated. I am going to make a few copies of this and have him and the JP sign each indivually, in case an original is required at the embassy or at immigrations in Australia & USA.

Hey there,

Im no expert on this nor have I had any great experience but to my knowledge when doing up an affidavit such as this one should have it all flowing as opposed to number format. I think this is so as nothing can appear or be accused of being added at a later date

I don't think the actual format of the letter matters as long as it says that your ex. knows and approves (or permits) the move.
