After i filed for my Mon last year, they first asked me with a "request for initial evidence" letter to provide pay slips and affidavit of support. I submitted all, so they granted my mom temporal work permit last year Oct, whiles waiting for the next stage. I just receive another letter this March 18 "Request for Initial / Additional Evidence (I-30)"

The five items they are requesting are

1. A baptismal certificate or other religious document with a seal of the church showing DOB and Place of birth.

2. Early school records showing DOB, my name and parents name and showing date of admission.

3. Medical Records such as hospital showing DOB, Place of Birth and parents name.

4. Census records: a state or federal record showing the name, DOB and place of birth and the names of parents.

5. Affidavits-Written statements sworn to or affirmed by two or more persons (other than the petititioner or beneficiary) who were livingsince that time.

Please my question is Why would they not ask for DNA test between me and my mom since most of the documents are no more or missing for decades? Can i do DNA test on my own with my mom who is also here IN THE USA and submitt it to them as the right thing to do?

For the #4 above, is it a census record in the USA or back home in my country Africa?

Finally is there any advice please as it is only possible i can get only two of the above requests.

It will help to understand the beginning of your process:

Did you file I-130 for your mom originally and what stage are you in now wne who is asking for the evidence?

That aside, you cannot submit DNA sample on your own unless the embassy has requested for it. They are askingfor census report from Ghana.

While they list all these things,satisfying one or two of them will go a long way. so why not start with affidavits.mind you they are asking for your own baptismal and birth certifcates. They are looking for something original that has both your names on it to show proof of relatinship. can you get a minister from her place of worhsip in Ghana to provide you with one that covers your lost baptismal cert.

Also have family members provide affidavit of your brith and relationship to her.

01/08/2013: Case # and IIN assigned ( file arrived NVC mail room 12/20/12)
01/09/2013: AOS invoiced and paid, DS-3032 emailed and mailed.
01/16/2013: IV invoiced &paid. AOS & IV mailed in one package(arrived 01/18).

01/28/2013: Case complete!!!
04/19/2013: Interview; APPROVED!!!!!
05/13/2013: POE; JFK


N-400: (3 months and 12 days)
Filed N-400 : 2011-06-17
Interview: 2011-09-27
Oath Ceremony: 2011-09-30

IR-5 for Mom Entire process took 5 months exactly
USCIS (22days)

mailed I-130 : 2011-09-30
NOA 1: 2011-10-03 (text & email)
NOA 2: 2011-10-25 (text and email)
NVC: (19 days)
Case entered and # assigned: 2011-11-18
NVC Case COMPLETED: 2011-12-07 ( 43 days from NOA 2 and 65 days from NOA 1)
Interview Date(Lagos): 2012-01- 23
Mom was late for interview
New Interview date: 2012-02-29 : VISA APPROVED

The reason they requesting for these documents is that ur birth was registered more than one(1)year since u were born and is not acceptable prove of relationship. Get affidavits from family attesting to the fact that they witness ur birth at the time and u are truly the child of ur mom. The other documents is hard to come by in Ghana so do get the affidavits. Thus what I did when i petitioned for my mom also. Good luck

Thank you so much kwakuoj and ndu26 for your great advice on this impotant issue of mine. Please i will go ahead and organize those documents i can lay hands on and then add the Affidavit. Please kwakuoj i will appreciate it if you dont mind to contact me at the e-mail miloukamis@yahoo.com so i can kindly ask you some few things since i do see some similarity in my case and yours please.Please ndu26 to answer your question, yes i filied I-130 and all other documents for my mom originally and she even had her Temporal Work Permit granted since last year Oct. One thing is that, I live in Wisconsin and she lives in New York. Initially when i filed they requested for initial evidence for my job and affidavid of support. I provided them all that they requested and so My Mom had her temporal job permit and also parole for traveling outside if she wants to., But never travel. The second Request for Evidence is what i am discussing right now with you all. They gave me 87 days to respond. I am so gratefull to you all, God richly bless you all.
