How do we know if he will need this form or not? It stated when my husband went for his medical in abu dhabi that he will need to do another varecella vaccine shot in june, 2013 but not now... do we need to do his medical again so we can send this along with the other things for AOS?

It depends. Do you have a photocopy of his DS-3025 (vaccination record) from his K-1 medical?

K-1 Visa Timeline AOS Timeline

- Aug 31st, 2011 - Mailed I-129F package - May 29th, 2012 - Mailed AOS/EAD/AP package

- Apr 13th, 2012 - Visa received - Aug 24th, 2012 - Green Card received

ROC Timeline

- May 19th, 2014 - Mailed ROC package to CSC

- Aug 8th, 2014 - Green Card received

N-400 Timeline

- Dec 29th, 2021 - Filed online. Got notice that biometrics will be reused.

- Now waiting...

It depends. Do you have a photocopy of his DS-3025 (vaccination record) from his K-1 medical?

No we dont have copy of that , when he went for his medical for k1 he only receive a form and that form is ds2053 showing his vaccination is this the form we can use to take with us to a doctor to use to fill out the ds-3025?
