This might be a dumb question? But how do you know if your medical is complete? meaning i paid for all the medical for my wife now and step daughter before they came from the Philippines. but when they got here i had to get like 7 more shots for my step daughter for school here in Texas. how do i know if i go to a civil surgeon and they won't rip me off by given my wife shots she really doesn't need? also does my step daughter have to go to the civil surgeon too? Thanks

I read that you wife and daughter have already had some shots here in the USA. As you know, most immunization/vaccine and/or booster shots are age-specific. My gal had a copy of her medical used for the K1 Visa, used it to get current immunization and vaccines at the local county health department, and when she visited the US CS she presented all 3 documents I just described. All the CS did was complete the medical form, I forget the name and USCIS form number, copies of her documents, went into another room and then came out with a sealed manila envelope for her it include with the AOS package.

my suggestion go to 3-4 clinics to have them check which shot does your wife and step daughter needed...some clinic don't even know how to really read the immunization record and tells u that u needed that shot. but as soon as u go to another clinic they have different answer/s lol just like what happen with us. we went from clinic to clinic. other charge more than the other OR some medical staff tells u that u needed those shot. but eventually u don't. then u end up paying extra for getting a shot than u don't really need. so be very careful with that!!

also, some clinic will tell u that u needed a physical test. before they could fill up the I-693...the trick just call some civil surgeon around your area ask them how much they will charge u just to transcript your wife and step daughter immunization record to I-693 form. some clinic will tell u that they won't do it without physical test then charge u A LOT ($175/$285/or even more it depend which clinic)...now, just let them know that the immunization shot is completed and have done physical test too...if you got lucky (usually the clinic don't ask for proof) they will just transcript the immunization record to I-693 form for a small fee ($45 or $50)

be very very careful with these surgeons, majority of them charges are unbelievable, it took my 4 days to get a good decent one, i was given 450 dollars price for my medical by 5 surgeons but i took my time and was lucky to pay 285dollars at the end, i met a God chosen surgeon. they will sometimes charge you for consultation, but just ask before you go ahead. once everything is completed and the report and all vaccination has been applied, they will ask you to sign and it will be sealed. please do not open. it will be given to you, i requested mine to be handed over to me and i dont my surgeon to post mine on my behalf

I wanna share my experience with you. Earlier this Feb,2013 I had my I-693 form done by a civil surgeon and he charged me $80 just in order to copy my vaccination record and stuff. My DS 3025 says my vaccination sheet is complete and in India during my K1 I was given MMR, Tetanus and some other shot and the rest were age inappropriate. I just didn't wanna take any risk only by sending DS 3052 along with my AOS so I started contacting civil surgeons from the USCIS website. There were a few Indian doctors on the list whom I called and they asked me whether I have insurance (Utter #######$) at the first place. I told them, we just need to fill up the I-693 and I don't need to go through any medicals as my medicals were done with in a year. They quoted me like $300 - 500. Finally, I found this shady place in San Jose, California namely Shasa Medical Center, where they told me it's $80. But they didn't know of the process unless I showed them online that I didn't need to go through another medical exam. They were kinda hell bent and they won't fill up any forms if I don't re-do the tests. Even the doctor asked me - oh I heard from a nurse that you are unwilling to take the tests and I was like seriously what the eff ? After a long argument and discussion I finally got my forms filled and signed up. So, just read the topics in VJ and double check it in the USCIS website, don't just spend an extra dollar. They will try everything to rip you off but you gotta hold on to yer every penny ey.

Just wanted to let you know what happen and who to see and not see in the Dallas,TX area. Went to a place closest to my house same place i went to the first time i did this about 5 years ago now, This is the place not to go to and i will report to the USCIS and there panels of Civil Doctors!!!!!! The name of the clinic is (Dr. Pedro Yen, Vital Family Practice Center 1900 Oates Drive, Suite 138, Mesquite, TX 75150) Their staff are a bunch of dumb @$$'s didn't know what was going on and didn't even care! I went into the office explained what i was needing they insisted on a physical so i said how much is that and the one i was talking to popped off and said $450 each! i said very loudly i might add your full of ####### first off you don't even know what your talking about and didn't even ask either doctors there about it. You are taking advantage of these people and I'm going to report this place!Man it mad me so mad! Got back out in the car and looked up this site trying to find were some had recommended to go and I found it! Here's who you need to go to for your civil Surgeon the name is (Dr. Alfonso Ramirez, M.D. 1121 West Jefferson Boulevard, Dallas, TX 75208 (214) 943-5773) I called them up and ask a few questions before i drove all the way over there from where i was. The first question was are you familiar with getting transcript immunization record to I-693 form. she said yes right there i knew i had the right place, Then i ask how much and she said just to the transcription would be $35 each and if they needed physical TB test and so on it would be $72.50 with the getting transcript immunization included. i read that somewhere on here about that and that was in 2010 it was only $72 then! So i said when could you see us see told me and we were on our way. got there ask for Stephine which was who i talked to on the phone and they explained everything to me i went a head and got the physical and TB and everything for both just to be on the safe side being I'm doing the AOS 8 months after they arrived. Total out the door was only $115.00 can't beat that with a stick! Got to go back today for the TB results and they will explain everything you are getting and then put it in the envelope and give you copies then i will mail it with the rest of my paper work for the AOS. I'm telling you this if you live in the Dallas,TX area go see them they will take good care of you! Thanks for everybody's help.
