Letter of consent help Please

Hi every one.

I had attended my interview on 12/16/2011 with my minor daughter(from previous marriage)for Cr1 and cr2 visas and got 221 g sheet to submit Court order regarding the custody of my daughter.The child is already living with me and the biological father is not interested in the custody so as per our law i couldn't even can't file for custody (until some one file for it or my ex- husband file for it)so a case of Appointing Guardian was filed and the court has appointed me the guardian of my own daughter and this case had been filed in the court after i got 221g sheet so all the facts court knows and even this 221g sheet was submitted in the court and every thing has written in the guardianship document that so and so date the embassy has issued one 221g sheet to get order regarding the custody of the child IT IS also mentioned that the mother (I) is now married to another person from America and wants to take her own child to U.S.A .

I got divorce as single party,declaration of Guardianship also as single party(ex-party) my ex-din't appear in the court in both the cases.I just wants to know

With this Guardianship document i have to submit the NO objection consent of my ex-husband?

Or The permission from the court to take my child with me to U.S.A.?

OR this Guardianship document is enough ?pls advise me which will be applicable in my case.

And pls send me the format of no objection letter from the other parent(i mean how to write and what should be included in this letter of NO Objection.
