hello all!

i am currently filling up the AOS forms and i don't know how to answer some parts. hope you could help me. Btw, my visa entry is K1.

Form G-325A

1. in the "Family name", I used my husband's last name, then in the "Middle name" I used my surname before I got married. Am i correct?

2. the "A#" is the number found at the back of the I-94 card, right?

3. in the part asking "This form is submitted in connection with an application for", what box am i gonna check?

06/25/2008 : Met for the first time

07/25/2011 : Met for the 2nd time, HE PROPOSED!

04/06/2012 : Sent I29-F Package

04/09/2012 : NOA1 received

10/03/2012 : NOA2 (no RFE)

11/15/2012 : Med Exam @ St.Lukes (scar found on xray film)

11/19/2012 : start of my 3-day sputum exam

11/21/2012 : 3 smear result is negative but have to wait 2 months for culture and sensitivity test result

01/21/2013 : PASSED the med exam! smile.png

01/28/2013 : Interview at USEM(7:30AM)-- APPROVED

02/02/2013 : Visa on Hand

02/25/2013 : USA entry

03/12/2013 : Got Married, got SSN after 4 days

04/08/2013 : Filed/sent AOS, EAD, AP

04/22/2013 : NOA1 letter received with biometrics appontment

05/15/2013 : biometrics

06/25/2013 : received the EAD/AP combo card

I'm part filipina so I thought I may be able to help you with this.

1. In the family name - yes use your husband's last name unless you plan to keep your last name. No, the middle name is for your middle name you list on your birth certificate.

2. Yes this is correct - if you're unsure it can also be found on your NOA2 notice.

3. "Status as Permanent Resident"

14 Jan 2011 - Fiance sent me a message on a dating website

26 Jan 2011 - Had a 5 hour long conversation that changed my life!

05 Feb 2011 - We decided to be an exclusive relationship

29 Jun 2011 - We met for the first time. It was exactly as I knew it would be!

14 Jul 2011 - He had to leave to go back to USA

17 Jul 2011 - Booked flights for me to go to USA for 3 months on VWP

29 Nov 2011 - Arrived in USA

25 Dec 2011 - HE PROPOSED!

21 Feb 2012 - Left USA for Australia

16 Apr 2012 - NOA1

08 Aug 2012 - RFE

11 Sep 2012 - CSC received RFE documentation

11 Sep 2012 - Did Police Clearance

18 Sep 2012 - NOA 2

26 Sep 2012 - NVC sent case to consulate

16 Oct 2012 - Packet 3 received

18 Oct 2012 - Sent DS-230 off to Sydney Consulate

01 Nov 2012 - Medical completed

07 Nov 2012 - Packet 3 Sent to Consulate

08 Nov 2012 - Packet 3 Delivered

15 Nov 2012 - Packet 4 Received

18 Dec 2012 - Interview APPROVED!

28 Dec 2012 - Entered USA POE:- LAX

09 Jan 2013 - Married

YES, PROVIDED YOU ARE NOW MARRIED TO THE USC AND WERE MARRIED WITHIN THE 90 K1 VISA PERIOD.

2. the "A#" is the number found at the back of the I-94 card, right?

YOU USE THE ALIEN NUMBER ASSIGNED TO YOU ON ALL CORRESPONDENCE FROM THE USCIS.

3. in the part asking "This form is submitted in connection with an application for", what box am i gonna check?

YOUR APPLICATION FOR A K1 VISA / FIANCE VISA.

To answer your #1, My wife used her last as a middle, no issues. We felt it showed continuity between her name on the passport and new married name on green card. She has not had any issues and we are pending ROC, traveled internationally and domestically etc.

In Arizona its hot hot hot.

When Chinook filed for AOS, she used my surname as her new surname. Also, she used her maiden surname as her new middle name.

She did the same thing with her Social Security card, her driver license, and everything else. No problem.

Chinook is now a U.S. citizen.
