VJers,

My wife came from Colombia last December (we had the K1/K2 interview in late October), we got married in February and now I'm in the process of filing for her and her son's AOS. While looking through all the paperwork, it seems the I-693 she was given in Bogota was left behind or kept by the CBP Officer at the PoE, bottom line we don't have it. Will those pose a major problem? Do I even need to include that with our AOS package since it has been less than 6 months?

In looking at the AOS guide posted here it says: "If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination." ...but what happens if I don't have it? Do I need yet another medical exam now?

She never HAD an I-693. That's a form completed here in the US.

Do you have a copy of her immunisation record?

Sorry meant to say the DS-3025, which is the immunization records given by the panel physician in Colombia. I guess I just answered my own question...need to see another doctor and get the new I-693...damn, more delay....grrrrr

The original DS-3025 is always in the sealed envelope given at POE, with other medical forms filled during the medical visit.

The thing is some doctors give a photocopy of that DS-3025, and some don't.

Do you know if her vaccination record is complete? If you know for sure that it's complete and if your wife happens by any chance to know for sure that the doctor didn't the box saying her vaccination record is incomplete, you can try to send her AOS package without anything.

If you have no idea or if she's missing vaccines, she will have to see a civil surgeon to have the missing shots and I-693 page 1 and the vaccination page complete. Does she have a record of all the shots she got in her lifetime?

You do not need a new medical exam, and the vaccinations required for AOS are: MMR, Tdap/TD, varicella(or a history of chickenpox), and a flu shot(only during flu season-Oct. thru Mar.). If you wait until after March 31st, you will not be required to get the flu shot.

We were not able to find a Civil Surgeon willing to fill out Immunization DS-3025 only, had to pay $200.00 to have the Exam done again, also some more shots.

A civil surgeon in the US would not fill out a DS-3025. That form only comes from the overseas panel physician at your K-1 medical. A civil surgeon fills out the I-693. You are not required to get another medical if yours was done within the year prior to filing for AOS. The civil surgeon needs to fill out Page 1 and Page 5(the vaccinations sheet) of the I-693, seal it up and have you mail you that in with your AOS paperwork. A lot of times it can be very hard to find a CS that will not rip you off and insist on a new medical or overcharging you for one anyway. Some have to call many different ones before they can find one. We had to drive 2 hours out of our way to find one willing to do only the vaccination transcriptions that were needed. Some folks like you just give up, give in and pay for the exam to get it over with.I still wish the vaccinations would be required at the K-1 medical for everyone so the confusion and hassle of finding a CS and dealing with getting parts of the I-693 filled out would not be necessary at all.

Same here, we are not able to find anyone that will complete the documents and only do vaccinations. Those who have found one are very lucky. We just made a copy of DS-3025, and made a copy of record of flu shot, and sent everything in.
