form DS-3025??

I just got my interview date, finally! But am not sure what this form is and whether it was in the package sent with my medical results, which I didn't open of course. Any ideas? Do I need to contact the doctor?

this is what the form from the consulate says:

Please ensure that the panel physician provides you with a copy of Form DS-3025, Vaccination Documentation Worksheet as part of your permanent health record. You should hand-carry this document with your sealed visa package and any other medical paperwork. If you did not receive a copy of the DS-3025 Vaccination Documentation Worksheet we can provide one for you at the time of your interview.

I did not see what number my form was, but the Dr. gave me a copy of my vaccination sheet along with my sealed envelope. You need this for your AOS I have heard. It sounds like they can give you a copy at the consulate.

12/07/11 Sent AOS package

12/09/11 AOS package delivered

12/14/11 Received NOA for I-485 and I-765 (we're not applying for AP)

12/22/11 Received letter for Biometrics

01/04/12 Biometrics in Phoenix

01/31/12 Received email update from USCIS saying that we've been scheduled for an interview

02/03/12 Got an email saying card production had been ordered for my EAD...now just to wait for the card...

02/09/12 Received email saying EAD card has been sent

02/13/12 Received EAD card :D :D :D :D

03/06/12 Interview in Phoenix :).....Approved :D

03/14/12 Received Green card in mail.....

....no more USCIS until December 2013

As above. You need to submit a copy of this vaccination checklist with your I-485 petition to adjust your status.

18 Dec 2012 - Interview APPROVED!

28 Dec 2012 - Entered USA POE:- LAX

If you are asking the consulate for a copy of the DS-3025, you should go ahead and ask for a copy of the entire medical. They will give it to you if you ask (they gave me a copy). It could come in handy.

Ditto to the above. I have mine too (which I'll be providing to my doc here in the US as well).
