Hi, I am trying to get everything finalized to send off my wifes i-485 and corresponding documents but I have a couple of questions.

1. When filling out her last name do I use her new married name or do I use her maiden name?

2. It is also asking for another G-325A. Do I send the one that we already used to get her K-1 visa or do we fill out a new one? Do I fill out one for myself as well?

I am new to this so any info would be helpful. Thanks much!

1. I will use her married name, and add all supporting documents showing name change through marriage.

2. I will fill a new G-325A. Since some time has passed between when you filled her K-1 and now. Yes fill one G-325A for yourself too. Good luck!

No, it's only one G-325A for your wife only. It is her AOS, so if she decided to change her last name to yours, she has to fill in the I-485 with her new last name.

If she changed her name when you married, then she would use her married name on all the AOS forms.

No G-325A for the USC. Only the foreign spouse is adjusting status. All the forms are filled out by them except the I-864. You cannot re-use the G-325A from the K-1 as things have changed, such as name and address.
