Hello All,

We are preparing all of the necessary paperwork to send in with the I-485. My wife had all of her vaccinations while in Vietnam prior to entering the US. She has a copy of the DS-3025 where it is checked that the vaccine history is complete and all requirements have been met. The DS-3025 is signed by the physician in Vietnam. She was also given a copy of her immunization record where a complete list of all of her vaccinations were recorded along with dates. All of her vaccinations were done within the one year requirement.

So, my question is this.... Do we have to fill out page 1 of form I-693 and have a civil surgeon sign it? Or can we simply provide copies of the DS-3025 and immunization record as all requirements have been met?

Just send a copy of the DS-3025 (keep one for you just in case). Many people did it and got no RFE (I am one of them).

Just a question though: the list of her vaccinations and their dates are also on the DS-3025, right?

Just make a copy and add it to paperwork.

Yeah, there are dates of the vaccinations on the DS-3025 also. However, she also received a copy of an immunization record from the physician in Vietnam because she had to go back for a second round of vaccinations after they had already gave her a completed DS-3025 form.

To be on the safe side I used a Civil surgeon to transcribe the ds-3025 to i-693. i figured for 26 bucks it was worth it not to stress over a rfe. Note: a lot of civil surgeons try to rip you off by telling you you need new physicals and so on. I called several in a my area before I found an honest one .
