I-693 Vaccination Document DS-3025

Hi everyone,

On my wife's DS-3025(Vaccination Documentation Worksheet) she has her "Td" vaccine marked as "Insufficient Time Interval," with all the other vaccines completed or "Not Age Appropriate."On the bottom for part 2 the physician marked off "Applicant does not meet vaccination requirements for one or more vaccines and no waiver is requested." Here's my question, do we have still need a I-693 Vaccination Transcript completed by a Civil Surgeon?

What happens if at the time of the transcript there is still insufficient time interval?

The doctor checked the wrong one. I am not sure why they marked it that way. Should have eligible for blanket waiver. Find a civil surgeon to transcribe them onto the I-693.
