Not so sure about work from home, specially since all the information they get from us is privacy related. But then, we can always wish that they can :)

Champi, I noticed that your case is also IR5. Here's what my lesson: We have to send them as many as docs, even some do not make too much sense. I was not born in America. And on our consulate site it says I have to submit my Naturalization form instead of my birth certificate since the former proves I'm USC, not the latter. But however NVC still sent me an RFE for my birth certificate. And also there's a very ugly form, actually not a form at all, with one page, have to put on some information, like case #, category, beneficiary's name, address. Even I'm in EP, and all the information is obviously in the coversheet and docs I've sent, I still have to submit that... So, in brief, just be careful and make sure to prepare everything. Maybe IR5 is more tedious than IR1 etc. God bless you!

HI Grace2013,

Thank you so much for the information. It is actually my husband who is petitioning for his mom. But I assume your advise is related to DS-230 package? My husband was naturalized last August. However, we do plan to send following information. Let me know if you think I should add anything to this list. Also, the ugly form that you had to fill out, is it specific to China or is it required for all IR-5 application regardless of where beneficiary is from?

0. Cover Letter

1. Bar Coded Cover sheet

2. 2 Passport Size Photos of the Beneficiary

3. Signed form DS-230 Part 1 by the Beneficiary

4. Unsigned form DS-230 Part 2 by the Beneficiary

5. Original Birth Certificate of the Beneficiary

6. Original Police Certificates of the Beneficiary

7. Photocopy of the Passport’s Biographic Data Page of the Beneficiary and PCC stamp

8. Sworn affidavit by aunt and grandmother (this is because marriage certificate of beneficiary is not available)

9. Original Unavailability of Marriage Certificate from registrar office (this is because marriage certificate of beneficiary is not available)

10. Death certificate of Petitioner’s father

11. Original Petitioner’s birth certificate

12. Original Petitioner’s marriage certificate from the Petitioner’s current marriages (only married once)

13. Copy of Petitioner’s Naturalization certificate

Copy of

5. Birth Certificate of the Beneficiary

6. Police Certificates of the Beneficiary

7. Photocopy of the Passport’s Biographic Data Page of the Beneficiary and PCC stamp

8. Sworn affidavit by aunt and grandmother (this is because marriage certificate of beneficiary is not available)

9. Unavailability of Marriage Certificate from registrar office (this is because marriage certificate is not available)

10. Death certificate of Petitioner’s father

11. Petitioner’s birth certificate

12. Petitioner’s marriage certificate from the Petitioner’s current and all previous marriages.

Why is it showing my wifes unmarried name on the payment portal? :help:

Should I WORRY?

Mine comes up with my unmarried name too! Hubby said he called NVC and they said it was okay. They will have a copy of your marriage certificate, etc to prove name change. They said not to worry.

Hey Champi, your docs look good to me! The p3 supplement (ugly) form is only specific to China, I believe. Hope your case will be completed pretty soon! :)

HI Grace2013,

Thank you so much for the information. It is actually my husband who is petitioning for his mom. But I assume your advise is related to DS-230 package? My husband was naturalized last August. However, we do plan to send following information. Let me know if you think I should add anything to this list. Also, the ugly form that you had to fill out, is it specific to China or is it required for all IR-5 application regardless of where beneficiary is from?

0. Cover Letter

1. Bar Coded Cover sheet

2. 2 Passport Size Photos of the Beneficiary

3. Signed form DS-230 Part 1 by the Beneficiary

4. Unsigned form DS-230 Part 2 by the Beneficiary

5. Original Birth Certificate of the Beneficiary

6. Original Police Certificates of the Beneficiary

7. Photocopy of the Passport’s Biographic Data Page of the Beneficiary and PCC stamp

8. Sworn affidavit by aunt and grandmother (this is because marriage certificate of beneficiary is not available)

9. Original Unavailability of Marriage Certificate from registrar office (this is because marriage certificate of beneficiary is not available)

10. Death certificate of Petitioner’s father

11. Original Petitioner’s birth certificate

12. Original Petitioner’s marriage certificate from the Petitioner’s current marriages (only married once)

13. Copy of Petitioner’s Naturalization certificate

Copy of

5. Birth Certificate of the Beneficiary

6. Police Certificates of the Beneficiary

7. Photocopy of the Passport’s Biographic Data Page of the Beneficiary and PCC stamp

8. Sworn affidavit by aunt and grandmother (this is because marriage certificate of beneficiary is not available)

9. Unavailability of Marriage Certificate from registrar office (this is because marriage certificate is not available)

10. Death certificate of Petitioner’s father

11. Petitioner’s birth certificate
