Annulment document and G325A

Hi, everyone!

My husband and I are about to lodge our visa petitions for me and my son. Both of us were previously married. For my part, I secured my annulment from Cotabato City. My question is, do I need to submit a copy of the Decision granting my annulment together with the authenticated NSO Marriage Certificate( for the previous marriage) showing the annotation of the annulment decree and a certified true copy of the Certificate of Finality of the annulment decree or would the latter two documents be enough to prove termination of my previous marriage?

I ask this because if I have to submit a copy of my annulment decision, then should I also state in the G325A form my residency in Cotabato?
