AOS | K1 | Last name question

My wife came over on a K1 and we were married. We are now applying for her AOS. Should the paperwork for the AOS be filed using her original name or my last name?

Her married name, if that's the name she wants to have.

8/17/10: INTERVIEW DAY (day 123) APPROVED!!

ROC:
5/23/12: Sent out package
2/06/13: APPROVED!
