Non immigrant visa application form

Hi VJ family, how are u all doing? i have a question for those who filled the k1 non-immigrant visa application, DS 156 form( i am asking for my cousin pls). He filled the form himself and he is the beneficiary. On page 2 of 2 of the form, number 39 says, was this application prepared by another person on your behalf? and number 40 says, Application prepared by NAME___________________________________relationship to applicant__________________________. Should he write his name as name of the person who prepared the form or is it his fiancee's(she is the petitioner) responsibility? and when they say relationship to applicant, do they mean his relationship to his fiancee or her relationship to him. Its a bit confusing. We just want to know if that section is for him to fill and sign or for his fiancee. Thank u.

That section is for when someone else fills in the form. If you have a lawyer or legal aide person , or maybe if you fill it out for someone that doesn't read english. If he filled out his own form just leave it blank. If the fiance filed it out you could have her sign it ( but they might want an original signature and that would delay things )

This will not be over quickly. You will not enjoy this.

Thank u very much NigeriaorBust, and yes, he filled it out, so i will ask him to leave that part blank as u have advised. Sorry,when u say an original signature what do u mean pls?

NigeriaorBust is right. if your cousin fills out the form himself then he should just answer, no, he was not assisted but if someone else is filling out the form for another then most times the signature of the person is required. I cant remember how the Ds 156 form looks. When my husband sent his DS 230 form to me, he stated he was not assisted in filling out the form but then i had to fill out the entire form again (he had signed blank forms) due to some additions and blanks we had to complete. Since i did not want them to question the difference in handwriting, I just stated he was assisted in filling out the form by his wife and added my details.

09/26/2012: Mailed I-130 from Nigeria( delayed by customs)
USCIS stage ( 66 days)
10/12/2012: NOA 1
12/17/2012: NOA 2 (case was transferred to NYC office 11/27/12)
NVC stage ( 20 days)
01/08/2013: Case # and IIN assigned ( file arrived NVC mail room 12/20/12)
01/09/2013: AOS invoiced and paid, DS-3032 emailed and mailed.
01/16/2013: IV invoiced &paid. AOS & IV mailed in one package(arrived 01/18).

01/28/2013: Case complete!!!
04/19/2013: Interview; APPROVED!!!!!
05/13/2013: POE; JFK


N-400: (3 months and 12 days)
Filed N-400 : 2011-06-17
Interview: 2011-09-27
Oath Ceremony: 2011-09-30

IR-5 for Mom Entire process took 5 months exactly
USCIS (22days)

mailed I-130 : 2011-09-30
NOA 1: 2011-10-03 (text & email)
NOA 2: 2011-10-25 (text and email)
NVC: (19 days)
Case entered and # assigned: 2011-11-18
NVC Case COMPLETED: 2011-12-07 ( 43 days from NOA 2 and 65 days from NOA 1)
Interview Date(Lagos): 2012-01- 23
Mom was late for interview
New Interview date: 2012-02-29 : VISA APPROVED

Timeline

Many forms you can sign and scan email, but others especially the I 134 or I 864 they need the original signed version and you must provide the form that was actually signed.

This will not be over quickly. You will not enjoy this.
