The proof of meeting must be within the last 2 years, not 2 years ago. I am going to assume you have 2 passports style photos of you and the fiance. Other than than, everything looks good to me.

Everything looks good, but it says to copy ALL pages of the passport, from the front to the back, including ALL blank pages. Good luck
