Form DS156 Question - US Manila Embassy

Hi...just a quick question please...in the VJ sample form...question no. 22 "When do you intend to arrive in the US' was left blank...in notice the sample was from an old form format...does this still apply today...would that question still be left blank?

please help...thank you...

Based on my experience... I left it blank. If you leave it blank they will ask you to fill the blank you left during the interview. That would be the safest way at this point.

Based on my experience... I left it blank. If you leave it blank they will ask you to fill the blank you left during the interview. That would be the safest way at this point.

do I still need to leave it blank even I have prospect date when I want to leave? I already filled up my DS 156 and put specific date when do i intend to leave do you think I need to fill up new one with blank answer on that part. Thanks

Another question is for ques no. 34 Names & relationship of persons travelling with you...

should i enter my fiancee's name? because our plan is that when i get approved, he will come here to pick me and then take me with him to us...

It's ok to answer the question.

Yes, you should enter your fiancée's name and her relationship to you.
