CFO seminar address

i am kinda confused... I am going to attend CFO seminar tomorrow. They said there is only one location for the seminar, the one in QUirino Avenue. others said it's in PRISM, infront of de la salle..

So which is which?

You need to go to Quirino Ave. Just had ours done 3 weeks ago

PRISM & St. Mary's were closed, only the office on Quirino Ave exists in Manila (like said there is an office in CEBU).

Do the children/k2 also have to attend the CFO? Ages 7 & 9 years old?

Exemptions from PDOS and Peer Counseling Seminar

For children 12 years old and below are exempted from attending the PDOS. They must, however, be registered, even if by proxy. The following additional documents are required if the proxy is not the legal/natural/adoptive parent of the registrant.

1. Original and photocopy of one(1) of valid identification card with photograph of the proxy
2. Special Power of Attorney authenticated by Diplomatic Post if parent/s/ or guardian/s are based overseas; or a duly notarized authorization letter executed by parent/s or guardian/s while in the Philippines.
3. Other documents that may be required by the registration officers.
   

The following emigrants are exempted from attending the PDOS and registering in person with the CFO, provided that they are registered by a proxy:

• Those who are 60 years old and above;
• Those incapacitated due to permanent or long-term ailments; or
• Those with mental health issues or psychologically-challanged.

In the registration of the above emigrants, the proxy is required to present the following documents in addition to the usual required documents:

• Medical certificate/report (except those 60 years old and above);
• Letter of authorization from the emigrant (if applicable to the case); and
• Original and photocopy of a valid identification card with photograph of the proxy.

No need. Just bring with you the passports of your children, IDS (if you have them), birth certificates to show that you are the parent, CFO fee of P400 each. :-)
