Where can I get the forms to apply for a visa for the Philippines ?

I believe the OP is looking for visitor visa or some such because they have already filing the I-129F.

OP - its best that you clarify what kind of visa you are looking to obtain so people can help you properly.

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

What kind of visa for the Philippines are you looking for? You do not need a visa to visit the Philippines. Upon arrival, your passport will be stamped with a 21-day visitor's visa. If you wish to stay longer than 21 days it would be in your benefit to apply for a 59-day visa before traveling to the Philippines. You can do this by applying to the Phil. consulate that services your area. Or, you can apply for the visa extension while in the Philippines. But, it is much easier and a bit cheaper to get the extended visa in the US before traveling to the Philippines.
