Weird question I-157

I was looking over the I-157 today that my fiance filled out and saw that

he filled out the street but not city and country of # 12 and # 17.

He insists he saw a sample form he googled and when I looked it up

it was as he said......just street address without city/country of employers and

schools.

I am unable to bring up VJ sample form, my old PC freezes everytime.

He said there is not enough space to add city/country.

What is correct ?

My wife put street, sub-district, district, province, and postal code - the complete address as is typically written on a mailpiece in her country. (The equivalent in a US address would be street, town, county, state, and zip code.)

She had to use both available slots in each section to do it, but as she only had 1 past employer and 1 qualifying educational institution that was not a problem. Had she needed more space she would have added an attached page to the form.

Thank you , that is what I thought also. Wonder why they would put

a sample form up without city/country.....
