Renewal of Passport and change of name

Hi everyone...

I am hoping somebody could help me here..My husband and I are planning to travel this Oct.to Phils. but the problem is my passport validity is until March 14,2014.So am I good to travel this Oct? or do i need to renew my passport before traveling? Also my last name is different from my passport and green card is there any issue for re entry to U.S with different last name on a passport and green card?My husband told me to renew my passport with my new last name so that it will match my green card and id's here in U.S but I did not report the marriage to the embassy after we got married(Dec.2010) bec. no phil.embassy in Kansas.So it confuse me if I should go ahead to renew my passport with my new last name before traveling and report my marriage at the phil.embassy?(but couldn't fine phil.embassy in Kansas) Pls.help.

I just checked into this yesterday. To change your passport to your married name will cost $110.00 I would think you will need the name correct on the passport to match your green card. It's probably best to get the passport updated. Good Luck and enjoy your upcoming trip :)

I would suggest to report your marriage and renew your passport prior your vacation. You still have enough time to do them. I made a research for you. Since you are in Kansas you can renew your passport and report your marriage at the Philippine Consulate General in Chicago. Here is the website (http://www.chicagopcg.com/forms_eppt.html). You can download the Passport Renewal Form here. I did the same thing last year but I went to Washington DC though because renewal of passport for Florida is in DC. My hubby and I drove to DC.

Here are the things you need to bring.

Passport Requirements (New Passport / Renewal of Passport : US$ 60.00 and Report of Birth/Marriage: US$25.00 and Self-Addressed Stamp Envelope: US$20.00 - I would suggest you bring cash. Personal checks, credit cards and personal money order are not accepted.)

PERSONAL APPEARANCE IS REQUIRED. All applicants, whether for the renewal of a passport, or those applying for the first time must appear personally at the Consulate General for data verification, picture taking, thumb mark/finger-printing and signing of the application form. Application by mail will not be accepted.

If applicant wishes the passport to be mailed back by the Consulate, applicant should BRING a self-addressed stamped envelope (with tracking number) such as Certified Priority mail, Express Mail or a prepaid FEDEX/UPS/DHL or USPS Express Mail envelope. Metered stamps will not be accepted . THE CONSULATE GENERAL ASSUMES NO RESPONSIBILITY FOR ANY LOSS THROUGH THE MAIL.

*** INCOMPLETE APPLICATIONS WILL CAUSE DELAY IN THE ISSUANCE OF YOUR PASSPORT ***

Only the following States are within the jurisdiction of the Consulate General in Chicago : Arkansas, Illinois, Indiana, Iowa, Kansas, Louisiana, Michigan, Minnesota, Mississippi, Missouri, North Dakota, Nebraska, Oklahoma, South Dakota, Ohio, and Wisconsin

A. RENEWAL OF PASSPORT

1. Accomplished e-passport application form.

2. Latest issued Philippine passport (the old passport will be returned to the applicant).

3. Submit the following documents:

a. Photocopy of pages 1-2 (data page) of the old passport and the page with the latest arrival stamp in the United States

b. Photocopy of the alien registration card (green/pink card), for U.S. permanent residents/ immigrants

c. If old passport does not spell out the applicant’s complete middle name submit photocopy of proof of middle name, such as Birth Certificate, Baptismal Certificate etc.

d. Photocopy of proof of actual residence such as driver’s license, state ID, employment ID, or utility bills, etc.

B. ADDITIONAL REQUIREMENTS FOR SPECIAL CIRCUMSTANCES SUCH AS CHANGE OF LAST NAME

4. For female applicant married in the Philippines who is using her husband's family name in passport for the first time, submit original/certified true copy and photocopy of marriage certificate duly authenticated by the National Statistics Office (NSO) .

5. For female applicant married abroad who is using her husband’s family name in passport for the first time, a Report of Marriage must be filed at the Embassy or Consulate which has jurisdiction over the place where the marriage took place. (See Report of Marriage Form and Requirements). Copy of the processed Report of Marriage should be part of the passport application.

Report of Marriage between Filipinos or a Filipino and a Foreign National

The marriage of a Filipino should be reported to and registered with the Philippine National Statistics Office through the Embassy/Consulate General having jurisdiction over the locality where the event took place. Please check the Consulate Finder or the Consulates Link for the states covered by the Embassy/Consulates General in the U.S.

BASIC REQUIREMENTS:

Four (4) original duly-accomplished Report of Marriage Contracted Abroad form, typed or printed legibly in black or blue ink, signed by both husband and wife, and notarized if sent by mail

Four (4) identical colored photos of the husband and four (4) identical colored photos of the wife, 2” x 2”, taken within six months before the date of application, showing a clear front view of applicant’s face without eyeglasses, with a white background. No sleeveless attire. Blurred or low quality photos are not accepted.

One (1) original or certified true copy and four (4) photocopies of the Marriage Certificate or Contract (original will be returned)

Four (4) photocopies of a government-issued identification card or passport of both husband and wife

Four (4) photocopies of Philippine birth certificate of Filipino spouse

Self-addressed return envelope, with appropriate stamps, via US Postal Service or private courier of choice, preferably with tracking numbers, if Report of Marriage is to be mailed back

Processing fee of $25.00 (non-refundable), payable in money order, bank draft, certified check or cashier’s check, and cash when personally applying. Personal checks and credit cards are not accepted.

For applicants residing in U.S. territories or other countries within the jurisdiction of the Philippine Embassy, applicants should enclose a treasurer’s, manager’s or certified check issued by a local bank that has a corresponding bank in the U.S., payable to the courier of choice, in U.S. dollars, to cover cost of mailing, and a corresponding self-addressed courier’s address label. Personal checks are not accepted.

Note: The Philippine Embassy/Consulate General assumes no responsibility for any delay or loss in the mail, or while the documents are in the custody of the courier service. The applicant should note the tracking numbers of all envelopes used and submitted.

Thank god I decided to keep my name and not pick up my husbands....life is a lot easier...I don't need to change anything whatsoever and can freely travel anywhere with my passport, and if wanted to, apply for all sorts of things both back in my home country as well as in the US without always worrying about how my birth certificate is different than my ID, or how my passport is different than my GC, my diploma when looking for a job etc. Phew!

Thank god I decided to keep my name and not pick up my husbands....life is a lot easier...I don't need to change anything whatsoever and can freely travel anywhere with my passport, and if wanted to, apply for all sorts of things both back in my home country as well as in the US without always worrying about how my birth certificate is different than my ID, or how my passport is different than my GC, my diploma when looking for a job etc. Phew!

Im from the Philippines as well...hi there kabayan..anyway with your concern about u travelling to Philippines with ur passport getting expired on March 2014...pwede pa yan....just be back before it expires. With your concern that you haven't reported yet your marriage..okey lng yan, just make sure when u buy a ticket, use the name on your passport then just bring your green card and documents like marriage certificate and/or marriage contract and ur good to go....hope this helps....

My wife has had no issues with married name different on her foreign passport vs us ID and green card. International tickets go in passport name, domestic go in GC and state ID name. She does carry a copy of her marriage certificate but had never produced it. On entry together all the CBP asked me was "is she yours" ans yep, after checking her passport and green card. I think he was surprised I went in the non us line with her.
