April 30 is our 2nd year marriage Anniversary

Their visa interview is on March 4

Lets say everything is ready 2 weeks after the Interview.

Do we have to wait for April 30 before they go here so they will have IR instead of CR?

Or will they be automatically be IR since it's under 90 days?

To get your IR1 you must POE (arrive in US) after your 2 year. Its when u lane in US. So yes you can interview and all but must wait until to be safe the day after your anniversary to POE.

In the Medical Examination form there's a selection for Visa Category. Can we select IR even though it's less that 2yrs but the POE plan will be after 2 years?

Sure trust me they will be checking all dates to make sure. Everything is determined at the POE that CBP person has the rubber stamper for CR1 or IR1 be nice to them.

No. The visa will be CR1. IR1 will be the status granted, if the entry is after the 2 year anniversary of your marriage. They don't always get it right, so check the card when it comes and if not right, get it corrected with an I-90 form and no fee.

IR1 is both a visa and a status. You will have CR1 visa and IR1 status. The visa category is determined by the interview date, while the resident status is determined by the entry date.
