Fiance Letter of Intent and Addresses

I'm sorry if this has been asked before or that I'm too dumb to figure it out on my own. I'm just a little confused by this letter. Is this something I need to fill out and sign? Or is it just something my Fiancee needs to fill out and sign? Because in the letter it says "...and intend to do so within 90 days of my arrival into the US using the K-1 visa." If I am suppose to sign this too can I just replace "...my arrival..." with "...her arrival..." and if this is something we are both suppose to turn in. Will I turn mine in first with the rest of the packet and then she will turn in hers later?

Also where can I find the correct filing location and embassy addresses? I am from Kansas and my Fiancee is from Caloocan City Manila Philippines.

Hi there, the address wont matter at this point what we did is that we wrote the address on where we sent the packet which is the Texas lockbox facility. Letter of intent should be submitted by both parties (You and your fiancee should make each copy and both should be dated and signed) and as for the Address here it is :
