Help with example affidavit

Hey all, I'm trying to put together the affidavit using the sample that's been posted here on visajourney. It's a little bit confusing to me, could someone please explain what the areas that I have highlighted in red mean? I need to get the ball rolling on this because their K1 expires this March. The new form comes out 1 February. Also, will my friends need to provide their SSN?
