Can the doctor at the medical also give out I-693A form, I mean can he transcribe the

vaccinations to that form or do we have to wait to get it done here for the AOS?

Just thinking ahead to see if we can save time and money.

You have to get the vaccincation supplement done here after landing in the U.S.

My I-693A looks exactly like my vacc supplement form that was done by a US-appointed civil surgeon in Canada... but we had to pay $50 USD to get the info transferred here. <_<

I just sent the vaccination suppliment, never saw a civil surgeon here.

*January 24 2006 - mailed in I129-F petition

*January 25 2006 - I129-F received at CSC

*January 30 2006 - packet returned.....arggggggggg we forgot one signature!!

*January 31 2006 - sent I129-F back to the CSC, hope we did not forget anything else

*February 1 2006 - I129-F received at CSC again

Hmmm I arrived here in the U.S. last June on my K3 visa and filed immediately for AOS last July..

Then the next step is to complete the vaccination (the Td shot was not given in the Phils). We scouted for a designated civil surgeon (the list is in the USCIS website) nearby our place. Take note, the fees they charge may vary a lot- from $50 or less to a whopping $350!!! You will have to inquire through phone from the list. Some (at VisaJourney) say that only a transfer of the immunization record is required, and no further Physical Exam is needed as you had yours at St. Luke's. But some of the designated surgeons wont agree to this. They will ask for another HIV and Syphilis test and a tuberculin test in addition to doing another P.E. and giving the TD vaccine. As for me, I agreed to having the P.E. done again (for $150) and my TD shot done (another $50+), as I thought (from a M.D.'s point of view) that it would be silly for a M.D. to just transfer immunization data from one form to another and affix his signature.

The medical exam and immunization record given by the designated civil surgeon (this will be in a sealed envelope) will be submitted together with the other documents (including the I-864) when applying for adjustment of status. I-485 can be downloaded from the USCIS website.You may apply for EAD and I-485 simultaneously if you want, you send both in one package.

Just do the medical & avoid risking that darn R.F.E.

I am just wondering if I don't necessarily have to see a civil surgeon here and pay for him/her to merely transcribe the document.

You probably sent the vaccination worksheet not supplement. The supplement you get in the US, the worksheet you get out of the US.

Yes, others have also used the DS-3025 Vaccination Documentation Worksheet from the visa medical exam for adjustment of status and have not received an RFE.

You don't necessarily need to see Civil Surgeon if your DS-3025 shows that you've either had all required vaccination or that any you haven't had should be waived as medically inappropriate.

Maybe, maybe not. Some people have had to, others like my wife have not had to.

We were able to have the I-693 done for $25 and it was real quick. If you decide to do it, phone more than one doctor on the list and see what kind of deal you can get, after all it's just copying a piece of paper.
