Hello,

The instructions for the I-485 state that a medical exam by a civil surgeon is required unless the applicant is filing from a K1 status because the medical exam was already completed as a requirement for the K1 visa. All that is needed is the vaccination supplement. So, we went to the civil surgeon yesterday to get this done. The nurse said the fee would be $550 for the exam and the vaccinations. After much discussion, the nurse was not aware of the K1, and that only a vaccination supplement was required. The clinic wanted $250 for the vaccinations alone. So we left the clinic with nothing done.

So I wanted to ask this group a couple of questions:

First, we don't have a copy of the I-693. When we arrived at customs we handed over the medical envelope to the customs official, he opened it, looked at it, then put it back in the envelope. He kept the envelope and never gave us a copy of the I-693. Do we need a copy?

Second, we have a copy of Nataliya's vaccination record translated into english, so we can take this to the surgeon. I understand we need to see the Surgeon first before we get the vaccinations that she is missing?

The nurse for this civil surgeon would not accept a vaccination record from a county health office if we were to get the vaccinations from there. Unfortunately, the closest civil surgeon is over 1 hour drive away, so this is turning into a really big hassle (time away from work, money etc). Can anyone shed some light on this requirement? Anybody know a good civil surgeon in Georgia, or Virginia who won't charge serveral hundred dollars for vaccinations?

I can only tell you what I did. She had a vaccination worksheet (DS3025) that she received from her docotr's exam in Colombia. I only sent that with the I485. I didn't get an RFE so I didn't bother with getting an I693. Our interview was on Feb 17th and nothing was asked. Good Luck

I didnt need any more shots. He just had to copy over from my worksheet from my medical I had done in Canada.

Still waiting for our cards...Had to file I-90 as they sent them to the wrong address.

March 9th, 2010, Received GC that has been lost in the mail for 10 months. Still waiting for my son's that is lost as well.

Filed Waiver for my son's 10 year GC and it was approved. He finally received his GC after its been missing for 2 years.

Thanking God this is over for 10 years.

You need to go to a surgeon/physician authorize by the USCIS to do the shots. Click the link below to locate the nearest surgeon in your place

http://www.uscis.gov/graphics/exec/cs/index.asp Bring also with you the copy of her shots and explain to them that what you need is only her vaccination supplement since she came as K1. They will release her vaccination in a sealed envelope (don't open it).

I would just like to say gl, it seem that most of the Doctors on the list a huge rip off artists...

from what you said it looks like you might just have to go and get them done at the civil surgeons, although why they would not accept records from a county clinic has me beat.. then you would only need the civil surgeon to fill in the form.

In my area there is only 1 civil surgeon on the list in a 3 hour radius, I phoned about my supplement and was told that it would cost me $150. This is with my vaccination records from the medical I did overseas so all that he would need to do is check that form and fill in his one. I sent my original one in without the supplement from him. If I get a RFE for it will do it then, but Im not just going to fork out that sort of money for a piece of paper that will take about 30 secs to fill in.

anyway good luck and Ihope you can get it sorted soon.
