When I did my POE at the Ambassador Bridge, the border agent didn't give me a copy of my medical (or any copies of anything, really) - he said the entire package would get forwarded to USCIS, and they would have everything they need when I file for AOS. I didn't think that was correct, based on what I've read here... but I asked a couple of times, and I didn't want to push it. Now, I'm getting ready to file AOS, and I'm not sure what to do... I have all of my vaccinations (and I have the public health record of them, but I don't have any record of them being transcribed onto official USCIS paperwork) - do I need to find a civil surgeon to transcribe them here in FL, before submitting AOS?

They keep everything in the sealed envelope and it is all sent to the USCIS. You should have been given your own copy of the DS-3025 vaccination sheet at your K-1 medical. You can try and request a copy from the doctor that did your K-1 medical. If you are sure the DS-3025 was filled out completely and all required vaccinations were marked off at the time of your medical, then you can go ahead and file for AOS without seeing a civil surgeon and hope it was done correctly and you get no RFE. You can also take your vaccination records to a civil surgeon and have them transcribed onto an I-693 to send in with your AOS paperwork. Finding a civil surgeon that will not insist on a new, unnecessary medical or overcharge you can be a pain to find. You will need to call around.

I didn't get the DS-3025 at my medical, but I do believe that the doctor filled it out correctly (I was watching him as he did it, and I think everything was noted properly) - so just to be clear, that means I can just skip the whole medical thing, because they *should* have it? If not, the worst that will happen is I get an RFE, and have to find a civil surgeon here to do it, or see if Dr. Mascarenhas in Toronto has a copy of it that he can send to me?

Yes. USCIS will already have a copy of the DS-3025 because it is included in the sealed envelope you turn in at your POE. It is not really a requirement to send in a copy of the D-3025 with your AOS paperwork. Sending in a copy of it is more of a reminder to them that all your vaccinations were complete. Add a note in your AOS paperwork stating that all vaccinations were complete but you did not receive a copy of the DS-3025 to include but a copy should be in your file anyway.

If worse comes to worse and you file without it and get an RFE, you can then worry about finding a civil surgeon to transcribe the vaccinations onto an I-693. Another medical is not required if your K-1 medical was done within a year of filing for AOS. Some civil surgeons will insist on one anyway and/or try and overcharge you. You will need to call around until you find one that won't. There is a post here on VJ called Civil surgeons who don't rip you off. You can check through it to see if any civil surgeons are in your area.

I'm in the same situation as you are, my wife doesn't have a copy of the medical examination and we were wondering what to send with the AOS package. I guess the question is: do we need to submitt for I-693 with the AOS package? Any ideas/recommendation?

No one gets a copy of all their medical forms. Most people are given a copy of the DS-3025 vaccination record at their medical. If you did not get all the required the vaccinations at the time of your K-1 medical and have them marked off properly on the DS-3025, then you need to get the missing vaccinations and see a civil surgeon to have your vaccinations transcribed onto the I-693.

blush.gif You're not alone. I struggled with this part when I filled out my AOS too.

I actually went to make an app. with a civil surgeon, (HAWAII, on Maui) was told I HAD to do another med. exam (by staff). Then told, they couldn't take me anyway, as the doctor was retiring! I'd have to FLY over to Oahu, see a Dr. in Honolulu. I cried a little, (after I left, silly girl) then REREAD the VJ link below. I've now got NOA's for AOS/EAD/AP. Bio app. Feb 5th.

Very helpful, when it comes to the 'medical (I-693) & vaccination record( DS-3025) portion of the AOS.

Insert from USCIS i-485(AOS) instructions: [Pg. 4 of 11] Initial evidence: Section 8:

Fiance(e)s : If you are a K1 fiance(e) ....... who had a medical examination within the past year as required for the nonimmigrant fiance(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your Form I-485.

Try and get a copy of your vaccination record/ vaccination supplement (DS-3025), from the doctor who did your medical examination for you visa.

Side note: I had my med. exam. done in the summer, so I was not required at the time to have a seasonal flu shot.

HOWEVER, I got a flu shot (with a record of it) before entering the US, (Nov. 1st), as a precaution because it WAS flu season when I entered, & it would be when filed my AOS packet.(received Jan. 4).

I agree with what Katie said originally. We also did not submit a vaccination "proof" with the I 485. We submitted the application in December and have had no RFE....yet. S
see the BOLD text below from the I 693 instructions:
2. What if I am a refugee and already had a medical exam overseas?
If you were admitted to the United States as a refugee and are now applying for adjustment of status under section 209
of the INA one year following your first admission, you do not need to repeat the entire medical exam you had
overseas, unless a Class A medical condition was found during that exam.
If a complete medical exam is not required, you only need to comply with the vaccination requirements. This means
you only need to submit the vaccination record and page 1 of Form I-693. Contact your State or local refugee health
coordinator to find out whether it may be possible for you to have the vaccination portion of Form I-693 completed by
a State or local health department. The State or local health department must also complete
Part 3
of the form.
If you were admitted as a:
a.
K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a
b.
K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and
c.
You received a medical examination prior to admission, then:
3. What if I am a K nonimmigrant visa holder and already had a medical exam overseas?
(1)
You are not required to have another medical examination as long as your Form I-485 is filed within one year of
your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or
if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with
the terms and conditions on the waiver.
(2)
Even if a new medical examination is not required, you must still show proof that you complied with the
vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part
of the original overseas medical examination report, you will have to have the vaccination report completed by
a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form
