Has anyone received a vaccination supplement from the overseas doctor doing the medical exam. Or does everyone have to take the vaccination worksheet to a civil surgeon to be transferred to the 693 supplement. We emailed the doctor in Brazil that did the exam, and he says that he does not have it.

11/09/05 - NOA1

Nov. 2005 - Visited my fiance in Brazil

02/02/06 - NOA2

Feb. 2006 - Visited my fiance in Brazil

02/24/06 - Packet received at NVC

03/20/06 - Fiance received packet 3 from consulate

04/26/06 - Fiance received appointment letter

05/09/06 - I flew to Rio

5/10/06 - Met fiance at airport

5/11/06 - Medical exam in Rio

05/12/06 - Interview in Rio - APPROVED!!!!

05/17/06 - Received visa in Rio

07/20/06 - POE Miami

10/16/06 - Married!!!!

01/10/07 - Sent AOS and EAD application

01/17/07 - NOA - Receipt

02/02/07 - Biometric appointment

02/09/07 - NOA - Transfer of case to California center

03/22/07 - EAD approved

05/10/07 - AOS approved!!!!

Hi-

Yes, you'll have to take your form to a civil surgeon in your area as listed on the uscis site and they'll transfer that info for you for a fee, I paid about $50, but it could be a lot less (or more...!)

November 30, 2006- CRIS email-Welcome notice sent

Mine cost about $30 in FL.

We paid $35.00 to have the information from the Vaccination Worksheet transferred over to the Vaccination Supplement. There were no vaccinations required.

Make sure you call around to get prices.... they vary significantly and some civil surgeons want to do the whole medical exam along with the Supplement. If you don't require the medical exam, find one that will just do the Supplement for you.

Didn’t you get a copy of the vaccination status from the doctor in Brazil? They are supposed to give us a copy of it, and then you take this copy to the civil surgeon in your area. For the chicken pox I told the doctor in Brazil that I had the disease when I was younger and he accepted my word for that, I was not asked to take the test here.

The Panel Physicians who give visa medical exams make their reports on DoS forms, they do not have access to CIS forms nor do they have the authority to fill them out.

It is not always necessary to take the DS-3025 Vaccination Worksheet to a Civil Surgeon to be transferred over to the I-693 Vaccination Supplement. My wife's DS-3025 was accepted as adequate documentation that she had received all necessary vaccinations, and many others have had the same experience. Not all, but many.
