CRBA Form DS_2029 - Item 12 * stuck *

Hello Everybody,

I'm applying for my son for his CRBA, and passport. I'm kind of stuck at item # 12 of the form DS 2029, which says:

List Precise Periods of Physical Presence in United States

http://www.state.gov/documents/organization/156216.pdf

The problem is I don't remember the exact Day ( not the month or year, of course I remember that ) :) when I entered the US for the very first time, and some other times when I left, and re-entered the land, that's from like 15 years ago. Knowing that, I don't have the passports from those times to see the entrance and exit stamps.

My question is :

Is the precise date ( exact day ) required for all times? knowing that I have the other precise dates from the past 5 years ?

Thank you all,

I'm in the same boat as you. I am filling out the DS2029 and passport for my son right now also. As far as question #12 I just look at my passport and fill out it that way. I'm not sure if it's the correct way though. My CRBA is schedule in the next couple of months. I won't know if I did it correct until after I've done them.
