Returning Resident Visa

I have a question in assembling and presenting the documents for returning resident visa at Mumbai consulate.

At the time of the application for returning resident visa at Mumbai, we were given the letter with the following statement. The attached document also asked for medical test documentation.

"Your application to be classified as a returning resident under section lOl(a)(27)(A) of the Immigration and Nationality Act has been approved. Before your returning visa can be issued. you must assemble the required documents and be scheduled for a follow-up inlerv icw. PI case read the instructions that you have been given and inform our office when you have assembled the required documents so that we may schedule you for an interview"

The appointement letter refers us to "visit http://mumbai.usconsulate.gov/ivforms.html". We were orally told to refer to packet 4 section in this link. Now, the packet 4 list is very exhaustive and we are wondering if

a. All Packet 4 documentats has to be assembled?

b. There is not mention of the fee payment - we have made the payment during the application review (for which this was accepted).

Dilemma we are going through is, should we go with what was handed out to us (which was just the medical test) or should we go through Packet4 list.

Answer to a. above is KEY - we have an appointment on January 8th and we really need advise if we have to gather all of the documents.
