My fiancee is a US citizen I am UK as is my son

Hi

wondering if anyone can help me, I have a US fiancee and we are in the process of applying for a fiancee visa for me and my son on to go the states to marry and eventually live there, I understand I need a notorized document from my x husband allowing me to take my son (has anyone done this and if so how should it read) and also when I get my interview in London do I take my son with me and does he require a medical, vaccinations etc?

any help and guidance would be appreciated
