Anyone know where the checklist for packet 3 can be downloaded?

I looked on London site but I guess I missed it. Steve got a letter from London saying they don't have his checklist and wont process his forms till they have it and it was sent, (they didn't send another one) So he called this am and they told him to download it off the Internet.
