This is the VJ guide on AOS, notice that it includes the I-693. FOr some reason I can't get USCIS page to open the I-485 instructions but if you read the form I believe it says something about the vaccination supplement. It says you don't have to have another medical if it's been within one year but to include at least the vaccination supplement.

I think you are confusing the 2 forms

I-693 is a medical

I-693A is the vaccination supplement.

From the Guide (the Guide covers AOS for both K1 & K3)

13. I-693, Medical Examination of Aliens Seeking Adjustment of Status. (If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required in this case. If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application.)

I think you are confusing the 2 forms

I-693 is a medical

I-693A is the vaccination supplement.

You're right I got them confused.

Don't you think they wanted I-693A and not the 3025 though? I thought some people got RFE's because they didn't send in the correct vaccination supplement?

People have gotten RFEs for the medical itself even though they've handed it in at the the POE. I don't think submitting a DS-3025 in lieu of a I-693A would suffice, but stranger things have happened. I'm sure there have been people that have gotten away with sending a DS-3025, a I-693A or even sending nothing but then again, many others have not been lucky enough and do need to submit something.

Another suggestion for the OP, call the POE and ask them where they sent your medical envelope.

Just in case the OP is unclear, the I-693A is filled out by a Civil Surgeon in the USA based on the info the foreign Panel Physician recorded on the DS-3025 or from other vaccination records provided if the DS-3025 is not available.

I am not sure if you took care of this yet or not..but my husband also got a RFE from CSC for medical test/vaccinations. I told them that he already had his medical, and the guy there told me to write a letter stating this, and just send in his vaccination records. He said it may or may not work. Since his medical was close to expiring, we just went ahead and took another medical exam. As soon as we sent in the information..his case was approved and he got his green card. It was a quick and painless procedure. Good luck!

I'm replying to everyone who has responded: Thank you guys for your help and knowledge on this issue I'm having. Through your brainstorming, I now see that I did not send in the proper form (I-693A). I will call a civil surgeon tomorrow and see how quickly we can get in to see them for this supplement. I wil also inquire tomorrow at the infopass appt. as to what else may be needed. Even though I was the "idiot" this time around, I refuse to be nice to USCIS and apologize for my disparaging remarks earlier :P .... I have no doubt that it will be resolved and the green card issued soon there after. In a lot of ways, I'm sure God was looking out for us by putting the crazy notion of making an infopass appt concerning our AP in my head...I'll be sure to let you guys know what I find out. Also, if we have time tomorrow, I may ask some questions concerning the whole process, just as an FYI kinda thing. Thanks to all of you :thumbs: I love you guys....

Hello,

I hope this helps your cause. Your problem is a trend at the CSC when cases are transferred. We received the same RFE back in July for a lost I-693. On our RFE it stated:

The medical examination that was performed as past of your visa application is not found in the file. For that reason, it is necessary to submit the following below:

I-693 MEDICAL EXAMImation: Please submit a properly completed and signed Form I-693, Medical Examination of Aliens Seeking Adjustment of Status. The medical examination must be conducted by a physician who is on the list of Civil Surgeons approved by USCIS. (You do not have to submit the immunization supplement that you have already submitted).

As you can see above the RFE clearly states it is for the 693 not the supplement. Which is completely frustrating as we were not given a copy of the forms from the CS either and turned everything in at the boarder as required.

As like you I could not stand anymore BS and incompetence by USCIS. I suggest that you contact your US senator’s office (We used Sen. Elizabeth Dole’s immigration attorney, Carol Armstrong). All of them have immigration liaison officers that can help in this type of problem. That is the path we took to fight, unfortunately for us they (USCIS) were still not able to find our “lost” medical paperwork and we still had to submit the forms again. Thankfully the CS only charged us an additional $50 to redo the paperwork. We sent in a reply to the RFE last week and are standing by for results.

As a final note if the RFE was for the supplement, we had a local CS transcribe the vaccination results onto the I693A that we submit with our I-485.

Please let me know what your course of action is and results. We are filing a formal complaint through Sen. Dole’s office once our case is approved and I would love to pass your info onto Sen. Dole in addition to ours.

Why would they need the I-693 from me, when we dont have that?...I'm assuming it was in the "brown envelope"...I sent the 3025 because that's all we have except the freakin' x-ray

because the rules say that you have to get the I-693 done by a CS here. All they do is transfer your Vaccinations and give you other vaccinations that may be missing.

People need to stop taking short cuts. If you look under the AOS guide I bet you will find something that mentions about this form.

No one is trying to take shortcuts. So I don't know where you are getting this from. Secondly, if it were clearly stated that I had to see a CS here to take care of transfering the vaccination record to another freaking form, I would have done that as well. Its pure stupidity on USCIS's part to require you to take one perfectly legal document provided by the CS in the home country (this case Kiev, Ukraine) and tranfer it to another one, requiring an additional $80 in fees paid to a doctor. (This is the I-693A) I called an emergency care doctor who is on the CS list to inquire about this today. Also, you obviously haven't read a damn thing between the quotes you replied to and the end of this thread. So, go back and read the whole damn thread, then make a comment.
