Proof of Immunization

Will they just be requiring the Yellow Immunization Records that the Dr. fills out at their office ... or will I require getting something more formal from public health?

I didn't have my record .. so the Dr. just gave me MMR and Tetanus shots (she said it was cheaper than a blood titre test) and filled out the card ....

My fiancee didn't have her medical records, but they took her word for chicken pox and tetnis. As far as MMR went, she had a blood test to show that she had her shots. If the blood test didn't take, she could've had those shots all over again. From what I've heard, the doctor you go to for your medical can do this, but they'll charger you. My fiancee used her health card and had the test done for free. The results were faxed back to the medical office and then her package was courriered to her within a day. Hope this helps.

You won't require anything more formal. The yellow card completed fully is enough. We had our medicals (my daughters and myself) and we all used the yellow cards with the dates etc and Dr info on it. You can give a verbal for the Chicken Pox...whether or not you have had it and when. The year was sufficient for me. Also, if you haven't had a tetnus shot in the last ten years get one. You need that too. Again, can be written on your yellow card.

My original yellow card from Canada with all my baby shots was transferred to a "World Health" card when we moved to the West Indies a few years later. I am unsure as to whether or not they will accept the original as they are transferred notes. My doctor simply reccommended just getting my shots again as I am nervous about ti, and I don't think it will cost me anything at all. Let's hope so.

The Doctor gave them to me so I sure do hope so!!! .. Just for info ... the MMR hurt like someone taking a live cigarette to my arm ... no wonder babies scream their heads off!!

Remember we beat this topic to death months ago. Everyone had different information given to them by different practioners etc. Some say it is just fine (including mine) and others say titres are the only way.

At the same time some charged for titres and others did not. Some took days for the results and some took weeks.

Some charged for the shots again and some did not.

Then there was the adivce added that we don't really need immunization until AOS and that it is a misconception with regards to the medical done before the interview. Regardless, at some point we all have to address it.

Go with whatever advice you are getting from your professional assuming you trust them....just my advice.

I ended up getting the MMR again based on the time it would take for me to get the titres back....weeks.

my doctor refused to give any shots until they ran a titre to see what I was already immune to. Didn't charge me a thing, and they don't agree that giving immunisations "just because" is a healthy thing to do. I didn't need anything except a tetanus/diptheria booster, all the rest they printed out on the titre results and I took that to the panel physician with the yellow card to show my tetanus shot.

I'm only just starting my K3 so my medical is awhile away... but, I'm due for my annual physical from my doctor and haven't had any immunizations for over 15 years.

If I get immunized at my next physical (in the next month or so), that takes care of this whole immunization process for when I do get my medical for my K3, correct?

I also had no proof of my immunizations, I knew I had them until Grade 9 which was the last shot I got. I knew I needed all my boosters so I called the doctor doing my medical and he said that was fine. He took my word for it that I had all of my shots as well as Chicken Pox and he gave me my boosters free.

I was missing info on one of my shots.

all it took me was a phone call to the clinic back home that I had been going to as a child (i understnad tho your file moves with you so whereever the last clinic you went to would have it)

They had it the next day.

There might be a form they would want you to fill out to give consent to release, but they can fax that to you also if you arent near the clinic.
