We are legally married , So it goes like this (Family Name)>> Married last name and for (maiden name)>>Her middle name before married.So

Your question is a little confusing. You are married and filling out the G-325A forms, and her Family name would be what she is legally going by right now. If she took your last name, then that is now her Family name. This part you quoted above, "Family name (For wife,give maiden name" is from the section about previous spouses. If she was married before, you are to list her maiden name there, and not what her legal married family name was when married to her ex-husband or what her name legally is now that she is married to you. Her names from previous marriages would be listed under "All Other Names Used" towards the top of the form.

She wrote down Petition, is that fine instead of 1-130 Immediate Relative petition. My idea is to Include a piece of paper and attach with its written of I-130.

I would just hand write '(I-130)' next to 'Petition'.

Mark the box that says Other, then on the line write i-130. No need to attach a copy of the i-130.

Thank you jay. I have another question the certificate of number in our naturalization certificate. Is that the right certificate # above the naturalization word? on other hand USCIS Registration No. was our permanent alien # right like Ex.( A123456789)
