Philippine DS230 for K1??

Can anyone advise if DS 230 is required for my fiancee at her interview in Manila?

Also if it IS required What should her answer be on question 45?

As of the New Instructions from Manila Embassy the DS230 is required and she is to fill out only Part I not part 2.

In regards to Question 45 it is stated DO NOT WRITE BELOW THE FOLLOWING LINE

The consular officer will assist you in answering item 45.

DO NOT SIGN this form until instructed to do so by the consular officer

This comes from the form itself.

I understand she will not fill out question 45 as stated on the form

I am just confused what would be the correct answer to the choices given

None seem to fit only OTHER

Would her answer be other K1?
