So what is the best time to change my last name? And should I use his last name when applying for Aos? So what should be done first, change last name on SSN card before applying for Aos? Do I need to get a new passport in relating to the new name?

Any question is about Flu shot. I have past my med exam back in Canada this September (for K-1), and at that time no other vaccinations were needed (that was during not flu reason so I never got to have a flu shot), so can I use the same report to apply for AOS?

Jan,2008 we met each other for the first time
March 11,2010, I flew to Dallas to visit him. We started dating.
September, 2011, we made the most stupid mistake we could have, sending out packet I-129 application without putting in a single page of supporting document.
January 12,2012, we received our declined petition letter after 5 months :-(
January 20, 2012, I flew to Dallas to prepare the packet and sent out the fiance petition
January 23, 2012, we received our noa1
May 30,2012, we received our noa2
June 13,2012, Our case was forwarded to Montreal
July 4,2012, I received RFE
July 5, 2012, I sent out RFE
July 9,2012, Montreal received my RFE
July 16,2012, I sent out my Packet 3 (I did not call Montreal to confirm if they have sent out my Packet 3)
July 25,2012, I received my Packet 4 in email
September 4,2012 at 9:10 am, visa approved !!!
September 6,2012, Visa in hand
November 17,2012, POE in Chicago

March 16,2013, AOS package sent out

March 22,2013,NOA received.

Apply for AOS with the name you want to use. If you want to use your husband's last name from now on, you should use it on everything starting now. That will the name that will appear on your EAD/green card.

You don't need to update your SS card first in order to file for AOS. Just write your SSN if you have one. In fact you don't even need a SSN in order to file for AOS.

You don't immediately need a new passport. If you travel internationally in the future (with AP or green card), book your tickets with the name on your passport, and keep a copy of your marriage certificate with you (+ your AP or green card for sure).

If you met the vaccination requirements at the time of your K-1 medical and that it was not flu-season, no need to see a civil surgeon (if your medical was done less than a year ago). Just include a copy of your DS-3025 in your AOS package.

So what is the best time to change my last name?

When you apply for your marriage license (need it to get married) it'll ask if you're taking you husbands last name...what last name you will go by once you're married. When you get married, you both sign, and you'll sign with your new last name.

Then you'll be issued a Married Certificate, and it will have your married name on it. Your name is change!

I got married in the state of Hawaii, and that's how it happened for me.

And should I use his last name when applying for Aos?

Yes. His last name, will be YOUR new last name, if you take it.

There's a question on the I-485/AOS form, 'other names you have gone by', you will put your maiden name, whatever you went by before marriage.

So what should be done first, change last name on SSN card before applying for Aos?

No rush on the name change for SSn card, you can't do this until after you get married, when you have your Married Certificate. You're asked to show your Married Certificate, when you do a name change. I just did this the other day.

I applied for my SSN 12 days after my POE, got married, got the married certificate, got my SSn card & went the same day to do the name change. (of course I scanned the card... so I know the number...) And now, new card is on its way with my married name.

A copy of your Vaccination Worksheet (DS-3025) is what you'll send in. They give you a copy with your med. exam.

Same situation for me, (medical done outside of FLU season), but I ended up getting the FLU shot the day before my POE. Just happened that 'Shoppers Drug Mart' was giving them, so I got it... just in case it was an issue later. You never know.

So I"ll submit the DS-3025 & Vac. rec. for Flu shot.

Since you aren't filing your AOS for a few months, it wouldn't hurt to get a FLU shot. Up to you though. Make sure you get a record of it, of course.
