Hey everyone,

We are currently working on the AOS process and realized we have no copy of the DS 3205. Does anyone know what to do here? My husband is from Cuba and they require them to personally get the vaccination record, so he can't send someone to get another copy.

You could try including a letter with your packet stating the original medical forms are in the packet he handed over at [name of PoE] on [date].

This may work, but only if he had all the required vaccinations marked complete at the time of medical in Cuba.
