hello just want to ask for those k1 k2 visa from phils...on the medical reqt on the i-485..did you just submit the one that st. luke issued to us (ds-3025 VAccination documentation worksheet)

???can i just photocopy it then include it to my i-485 coz im filling just within one year from medical in phils and same with my son (k2) any advice please....please help...thanks in advance//

Since you and your son had your medical exams at St. Lukes, then it's highly likely that you received all of the age-appropriate vaccinations already. In that case, you should be able to submit copies of the DS-3025's with your AOS packet.

However, some people are getting RFE's when they submit their DS-3025's. The RFE requires them to submit an I-693. In that case, they must have a civil surgeon transcribe the vaccination information from the DS-3025 onto an I-693. Then, the civil surgeon will seal the I-693 in an envelope and give it to you.

The problem with getting your DS-3025 transcribed onto an I-693 is that many civil surgeons don't know the process for us K-1'ers. Some civil surgeons mistakenly want to give you more vaccinations, and other civil surgeons mistakenly want to give you a complete new medical exam. So, you need to shop around and find a civil surgeon who understands the K-1 process and simply will transcribe the DS-3025 onto the I-693...and nothing more.

Chinook and I found a civil surgeon who transcribed Chinook's DS-3025 onto an I-693 for about $35-40. It took about 15-20 minutes at the civil surgeon's office.

Same here we went to a civil surgeon and had the 3025 transcribed to the 693 (that the civil surgeon sealed in an envelope) for $40, then sent that with the 485.

We also went to a civil surgeonand had them do the 693 sealed envelope. Abs still needed 3 shots as her 3025 said vaccinations incomplete. She needed 2nds on two part shots and another damm flu shot since it's the season now. I was mad about the flu shot since she had one 2 months previous, but what do you do?? I don't ever get them and she didn't want it either...

27 January 2012: Mailed I-129F

03 February 2012: NOA1( e-mail & Text)

03 February 2012: Check Cashed

NO RFE'S

22 June 2012 : NOA2 (e-mail & Text)

16 July 2012: Manila Case Number(by phone)

17 July 2012: Interview paid at BPI

19 July 2012: Set interview for Mid-Aug

23-24 July 2012: Medical St. Lukes(passed)

24 July 2012: CFO Seminar(had to go next morning for landline #)- PASSED

02 Aug 2012: Received e-mail from USEM our case is there.

15 Aug 2012: Interview at USEM - APPROVED

13 SEP 2012: POE Minneapolis, MN

27 OCT 2012: Married

19 NOV 2012: AOS package sent

05 DEC 2012: NOA's I-765, I-131, I-485

14 DEC 2012: Biometrics appointment finished(Walk-in..Was scheduled Jan 04 2013)

02 FEB 2013: I-131 and I-765 Approved

07 FEB 2013: USPS Picked up the combo-card

11 FEB 2013: Received Combo-card

21 FEB 2013: Transit Visa picked up in Chicago for Japan
