For the I-134. Is the I-134 the correct form for the petitioner or for a fiancee'(Applicant)? Also is it required the petitioner provide 1 or 3 yrs tax returns? I see 1 yr on the form itself, but am being given advise for 3 yrs.

The I-134 is the affidavit of support that is filled out by the USC petitioner/sponsor. Only the most recent tax transcript is required, but every consulate is different in their requirements and procedures. If people that went through Lagos tell you to include the last 3 years, then you should do that.

I was reading through that document you attached out of curiosity, and I actually noticed that it's asking for tax returns from last "3-4 years".

Current tax year is required all 3 is optional. But for Lagos you should have the last 3 years Tax transcripts and have at the ready if asked. Same if u have a co-sponsor. And have tax transcripts not forms and W2's.
