Do you put yes or no on the question number 36 of DS 156 form for K1 visa

36. Has anyone ever filed an immigrant visa petition on your behalf?

and also number 22. when do you intend to arrive in the us? what if I don't know exactly because I don't know when is gonna be my interview date what should I put there?

22: I wrote mine this way : "As soon after I receive my visa." Since I don't have a fix date yet. :)
