It depends. The medical is usually valid for 12 months, and no need for a full medical when AOS is submitted before it expires. In such case, you'll need to submit the copy of ds-3015 which is the vaccination sheet from your fiancee's medical. It needs to be complete(or incomplete with "waiver available" marked - such as if she has already had chicken pox and doesn't need a vaccination, or if none of the remaining vaccinations were age appropriate). If she still needs vaccinations, she'll have to see a civil surgeon to get the rest of the vaccinations needed however she can get the vaccinations from any doctor as long as she has proof to bring to the civil surgeon. THen it's transcribed onto a form and that has to be submitted with the AOS.

To sum it up: If she had all vaccinations and medical was done 12 months or less prior to submitting AOS, you can write in the cover letter as part of your evidence "Medical examination; Copy of vaccination supplement DS-3025 based on K-1 within the last year" or something along the lines of that.

Send check/money order in the AOS package(along with biometrics fee). Some get interviews, some don't. You'll know later on in the process sometime after having gone to the biometrics appointment. That letter is sent to you after your AOS package is received.
