Basically these are the main forms you will need to fill and you need to attach the supporting documents and passport pictures.

G-1145 E-Notification of Application/Petition Acceptance

I-130 (Petition for Alien Relative)

G-325A (Biographic Sheet )

I-485 (Application to Register Permanent Residence or Adjust Status)

I-693 (medical examination)

I-864 (Affidavit of Support)

I-765 (Application for Employment Authorization)

I-131 (Application for Travel Document)

But make sure you read the instructions for each form to guarantee you don't miss anything.
