2. in regards to "Dates of Service". it says "mm-dd-yyyyy" should i put only one date. or should I put 2 dates (e.g "From" date and "To" date)

1. Branch means "Army, Navy, Marine Corps, etc...or country equivalent, but since you said you were army, put "Army"

2. This depends... If you are no longer in the army, you would put the dates that you were in, for example mm-dd-yyyy to mm-dd-yyyy

If you are still in the dates would read like this mm-dd-yyyy to present.

Someone please correct me if I am wrong. My fiancee didn't serve in the military, so that question didn't apply to us.

Does it also include Thank you for your Service?

“If you know the enemy and know yourself, you need not fear the result of a hundred battles. If you know yourself but not the enemy, for every victory gained you will also suffer a defeat. If you know neither the enemy nor yourself, you will succumb in every battle.”
