How come it doesn't list the fee?

Or am I totally missing it?

Edit: NVM Found it on the direct page you sent me to rather than the page after that.

It will list the fee at the end of the application once you go through everything.

That one asks for weight and count of older siblings (weird)

I wonder if that part isn't required then?

Only the questions with a red star beside them are required for the guarantee of 5 days processing.

Did you get your husband's certificate ordered? I ordered long forms for myself and my 2 children online. Each cost $35 because it was the first time they were being ordered in that format. We, in Ontario, receive the small version usually and long forms are only needed for cases such as this. The more information you enter, for example weight at birth or doctor's name, the better. This helps them to verify that the person making the request is actually ALLOWED to make the request and knows a great deal of information about the person it is being ordered for. If you order it for your own child, you are likely going to have ALL the answers. If you order it for yourself (or you pretending to bee your husband when you order it) then you may not know all the details without being able to ask mom or dad. But either way, the more you answer, the faster they will process it. When I did mine, I was able to fill in every single line without anything being missed so it was processed that very day and in the mail the next day. I had it within 3 days of filling in the form online. When I spoke to someone on the phone about it he said that it was so fast because I was able to fill in every bit of information.

I hope this helps a little bit. It was actually really neat to receive the long form for my birth because it was hand-written by my mom when I was born so the form is actually a lovely copy of her own handwriting with all my information at birth. My kids' are typed because they're done on a computer now so not as much fun to see!

So, now I'm wondering if I ordered the wrong thing? I thought we just needed a birth certificate with our parents names on it?

that's what I'm thinking. I noticed a lot of people on here confused by this, our immigration attorney said many times "original certified copy" and that's what I ordered for sending off our I-130 back in January... I sent the long forms in for the kids and nothing for me since it's not included in the instructions. I sent PHOTOCOPIES of them and will have the certified copies, which is what the registrar general sends, to send in when requested.

The long form birth certificate that the Consulate requires needs to have your parents full names which means your Mom's maiden name listed. I don't have anything other than that on mine - my name, birth place and birth date and their full names. I don't believe there is anything else listed. Mine is from Quebec, though. Anyway, what the Consulate wants is both parents full names in addition to yours.

Here is the online form for Ontario...it does ask those same types of questions. For some I know you don't have to be exact (ie the weight at birth) but his mother's info does need to be correct.

You also need to have a guarantor. Which is someone in a professional career...doctor, lawyer..etc that has known the Canadian citizen for at least 2 years and can certify that they are who they say they are. You need their ID number for their profession I guess. It is crazy. WAY too much hassle for a document that says you were born at one time.
