Urgent! I-765 (EAD card) instructions

Hi!

I need to fill out a I-765 application form to renew my employment authorization card. I was able to download the form on the USCIS website but cannot download the instructions (it basically says that I need to upgrade to the latest Adobe Reader version. I did upgrade, and it still won't open. I tried on my husband's laptop too, and still nothing).

I need to fill out question 14 "who may file form I-765" and put the letter and number of the eligibility category. I can't do this without the instructions form.

Does anyone have the form available and can copy and paste the different categories for me?

Also, what else do I need to include in my application?
