Where do I find this? I don't beleive I have one, or is it inside of the Unopened packet?

I have a vacinations certificate which says it goes in conjucntion with the DS-3025.

Dyslexia strikes. The original is in the unopened packet. Your copy looks the same. The title says in conjunction with DS-2053 (medical exam form number). You should find the number DS-3025 in one of the bottom corners. It's the vaccination sheet.

No Rob, the original is with the rest of the medical forms in the brown envelope that you will hand over at POE. You have no access to the original but the USCIS will have it in your file unless they lose the whole brown envelope. You can make a photocopy of the vaccination sheet Knightsbridge gave you and send the photocopy with your AOS packet. it's more of a reminder that you want to use that to prove your shots. They will have the real, untampered with original in your file. That's the one that holds weight because it went sealed from Knightsbridge to the embassy to you to POE without any chance of you altering it. It's a chain of custody thing.

Make a photocopy to send. Keep what Knightsbridge gave you.
