It's been more than a year since I had the medical exam required for the K1 vis, so I need to have another one now that I'm applying for adjustment of Status. One place I called asked me if I had my immunisation records. I don't have these to hand, but I think I will be able to hunt them up; my question is, do I need to? I saw this in the FAQ thread:

All your medical information is enclosed in the brown envelope which was handed in at the POE. The envelope is then forwarded on to USCIS where it will be consolidated with the I-485 submission, once received.
Can the civil surgeon I see access that medical information?

There is also a name and address for a civil surgeon who will transcribe vaccination information to I-693 form for a fee; is that person the only one who can do that?

I apologise if this is very obvious and basic stuff, but I'm finding it difficult to work out exactly what is necessary for this exam. Any clarification will be greatly appreciated.

I would take the DS-3025 copy to the CS.

It can be expensive when having to redo. Good luck :)

Use the USCIS website to locate a civil surgeon near you. They won't be able to access the medical that was completed for the K-1 so you will definitely need to redo it. I think you should try your best to find a copy of the vaccinations you received or at least get a titre test completed. The civil surgeon may work with you to determine what vacations you've had/need.

Do you have a copy of your vaccination worksheet from the medical you've already had completed? It's called a DS-3025. You would have received this when you went back to pick up your medical results for the K-1.

The civil surgeon has no access to the paperwork you turned in at your POE. Those were sent to the USCIS and are on file with them. You need to get another full medical done, and you should find your immunization records or a copy of the DS-3025 from your K-1 medical. Without proof of your vaccinations, they will just give you all the ones required for AOS, MMR, Tdap/TD, varicella (unless you already had chickenpox) and a flu shot during flu season, which it is right now.

Call around to find the one with the best price. It can range from $150 to even $600. Find the best deal in your area.
